I give permission for my child(ren) to participate in a caroling visit with the North Madison Congregational Church to Peregrine's Landing nursing home in Clinton.  I understand that the children will be transported by school bus and supervised by their church school teachers during the trip.  Buses will leave the church at 9:45am.  Pick-up by parents is at approximately 11:15-11:30am at the church.  An emergency phone number where I can be reached, if needed, during trip hours (cell) is: ______________________.
